Frequently Asked Questions

Upon coming to our facility for your first appointment, a Patient Care Coordinator will go over the initial paperwork and explain your insurance benefits with you. The evaluating therapist will greet you to conduct the evaluation and discuss the difficulties you are experiencing. Together, you and the therapist will establish goals and the therapist will then form a personalized plan of care to achieve those goals. Once the evaluation is complete, treatment will begin. 

 Please note, patients are not guaranteed to see the same therapist for each appointment. 

For your initial appointment please have your insurance cards, photo ID, an updated medication list and the prescription from your doctor.  Please arrive to your evaluation appointment 30 minutes early to fill out new patient paperwork, or download the forms in advance HERE

Please dress comfortably and wear closed-toe shoes. Sneakers are preferred but not required. Prepare for a great session to get you moving better again. 

 

Treatments last approximately an hour and are specifically one-on-one with a therapist each session.

We accept most Major Medical Insurance companies for example Medicare, Humana, UHC, Aetna & BC/BS. Please note a valid doctor’s prescription is required when using your insurance. For a detailed list of insurances we accept please click HERE. 

Our scheduling is easy and we offer a convenient location and appointment times for you. It’s best to schedule in advanced especially during our busy season.
